We will discuss Neoliberal economics, capitation consensus (page 1-16), the RAND Insurance experiment, and other important topics AT A LATER DATE. "These vertical acquisitions are frequently framed as value-based care (VBC) investments. In primary care alone, it is estimated that $50 billion has been invested by these corporate entities in recent years, with similar activity in home-based and post-acute care. And private equity is lubricating these rollups, increasing investment in VBC-aligned companies by more than 400% between 2019 and 2021. All players uses an insurance to "funnel" the cash through their subsidiaries. I do not agree that the graph should look like a chain ring, as the funneling always STARTS with the insurance and then "trickles down" through the subsidiaries, but not always in a linear fashion. The overall "value" will be its earnings/profits. It is through the initial funneling that this produces the value added parts of subsection (or chain, in the graph). What this graph lacks, which the 10K and 10Q can furnish, is this: how much money do you think the funneling is occurring? My guess is Optum and UHG is crossing over their revenues in certain spots, since UHG is referring patients to Optum, which is under the same umbrella as UNH. As you can see, each year the eliminations is a higher percentage. In fact, it is now > 33% and approaching 40%. Or another way of saying this more clearly - we don't know why this is the case, but it seems UHG is having more and more patients being served by Optum, and so the eliminations revenue is higher % each year, which all goes under the parent company UNH. In fact, if you eliminate the eliminations, revenue of both UHG + Optum combined is higher than UNH - which cannot happen if the revenue comes from an outside source. We can even track its YoY trend. Let us turn to the profits section. Again, very similar story. Between 2021-2023, UHG + Optum profit is greater than UNH profits, which indicates that UHG is directing a certain portion of the profits into Optum, but they have to cross each other out during reconciliation for UNH's earnings report. The only time this was an exception was during Q1 2024 durring the Change Healthcare cyberattack, where Optum Insight's profit dropped by ~ 8.5%. Therefore 24Q1 the added total earnings was exactly 100%. What this means is that I personally think there is potential market crowding and market preference within the UNH group. We will need further conclusive proof from the DOJ antitrust investigation to ascertain whether there is a true market distortion practice occurring OR if Optum services are so good UHG must always choose Optum as their partner due to quality (OK, can I start laughing now??? HAHAHAHAHAHA). Furthermore, we haven't even discussed Home Care. "UnitedHealth is also moving into in-home care, with both primary and post-acute providers. In 2021, it bought PE-backed Landmark Health for $3.5 billion and recently merged Landmark with Prospero Health, after acquiring it in 2022. UnitedHealth also acquired LHC Group for $5.4 billion in 2022 and is currently seeking to purchase Amedisys, a home health and hospice company, for $3.3 billion. These home-based assets complement UnitedHealth’s 2020 purchase of PE-backed NaviHealth, which is one of a handful of post-acute “conveners” that have emerged within capitation-based models, particularly MA." As a conclusion, I believe there is definitely a way to cross review some parts to this dense position paper via 10K/10Q, and this is reflected within the financials of UnitedHealth Group. We will delve into the failure of Bright Health Group, the struggles of CVS/Aetna + the struggles of Humana at a later date. Suffice to say, vertical integration is the name of the insurance game, and unless something breaks the equilibrium, this will be the standard model for health insurance companies based on UNH's template. |

Eu sou um grande fã de Assassin's Creed desde quando joguei o segundo jogo em meu Xbox360, parece que se passou uma eternidade desde então, graças a retrocompatibilidade do Xbox eu tenho todos os jogos da minha biblioteca aí seguido rejogo os clássicos, e eu particularmente não entendo muito bem o que está acontecendo na indústria dos jogos ultimamente, eu sei que pode ser ingenuidade da minha parte ou pode ser simplesmente que eu diminuí meu ritmo de jogatina devido ao trabalho e estudos, mas é totalmente injustificável e é uma pena, deixe me explicar, prometo que vai fazer sentido. submitted by AIRCHANGEL to gamesEcultura [link] [comments] Uma coisa que eu sempre gostei em Assassin's Creed, não só a história a cultura a gente emergir num cenário que não existe mais até hoje eu jogo Origins para observar a cultura e hábitos do Egito antigo que eu sou muito fã, mas também a perspectiva que os personagens nos oferecem sobre a realidade deles na época o fato da gente jogar com uma pessoa de uma cultura alheia a nossa é algo que sempre me cativou, o jeitão italiano e maravilhoso do Ezio, a lealdade do Adé, o coração bom e o senso de justiça do Connor para com seu povo, a Kassandra falando Malaka a cada um segundo no jogo é maravilhoso e o Bayek que mesmo perdendo a fé no Egito ainda mantém a fé ao que ele é e ao seu estilo de vida. Isso sempre foi uma coisa que eu não ou só achava incrível mas também achava maravilhoso que assim eles conseguiam incluir todo mundo e não ofender ninguém. E quando não se tratava só apenas do nosso personagem eles criavam um personagem maravilhoso para representar a cultura onde o jogo se passava, meu caso favorito é o Yusuf no Assassin's Creed Revelations, até hoje é o melhor turco da história dos jogos, se eu tiver errado fico feliz em ver os comentários. O trailer do Assassin's Creed shadows está com 90% de desaprovação no Japão, não é metade, não é uma minoria, é grande parte do público-alvo que a Ubisoft tentou no país do jogo que ela vai retratar a história, e eles tem toda razão devido a esses argumentos que eu citei Assassin's Creed sempre foi sobre inclusão ao ambiente e sociedade do qual os jogos se passa. Tem um monte de coisa insana de bom naquele trailer como as armas, as armaduras, o ambiente é sempre maravilhoso em um Assassin's Creed, não tem como negar, até certo os jogos que possuem um aspecto mais feio em relação ao outros são perfeitos pois retratam a realidade daquele lugar, muita coisa naquele trailer tá incrível, outras nem tanto eu concordo tipo a irrealidade de certas armas em relação a outros trailers de Assassin's Creed que a gente viu não batem muito como as estrelas Ninja e as katanas furando uma armadura como se fosse nada. Mas sobre o que estamos falando, sim eu entendo que possa se referir a Yasuke, o daimyo de Nobunaga, que foi para o Japão a bordo de um navio português e acabou se incluindo na sociedade local, beleza se o nome dele for realmente Yasuke tá ótimo tá perfeito, a garota que eu ainda não sei o nome ela tá ótima ela não se tem nenhuma reclamação ela é japonesa, e representa a cultura local no trailer. Mas eu entendo a frustração dos japoneses e concordo com isso, nós estamos bem dizer desde a época do Assassin's Creed III esperando por um Assassin's Creed no Japão, tenho certeza que por mais que o Ubisoft não seja de grande importância em matéria de mercado de jogos no Japão, ela ainda possui muitos fãs lá assim como em qualquer lugar do mundo. E da mesma forma que se houvesse um Assassin's Creed, sei lá na revolução Farroupilha, na época do cangaço ou no Brasil República, nós ficariamos enraivecidos se o protagonista fosse um inglês, um francês, um americano e nós não fossemos representados. Os japoneses que gostam de Assassin's Creed esperaram muito tempo pela vez deles, e eles não vão ser representados por um deles, da etnia deles, como Assassin's Creed sempre foi e sempre defendeu, uma tradição que fez ela ganhar uma citação honorária do Museu Egípcio do Cairo por fazer a melhor retratação interativa do Egito Ptolemaico já vista, e essa tradição é quebrada. Isso é ofensivo para qualquer povo, e é totalmente compreensível. É aquela história, não faça o desejo para o outro que você não quer que aconteça contigo. Ignorando nessa conversa o fator de preço que a gente sabe que é um absurdo irrealista ou cobrar quase metade do salário mínimo por uma mídia digital ainda. Mas uma coisa que também não se dá para negar nessa história toda é o fato que a qualidade dos jogos da Ubisoft decaiu muito, bem dizer o último ano descente da empresa foi há 5 anos, com FarCry 5 (maravilhoso), AC Odyssey (que para mim é para Assassin's Creed que Metal Gear Rising é para Metal Gear, um bom jogo mas nada parecido com o resto da franquia) e de lá para cá a Ubisoft simplesmente caiu, nós vemos esse efeito nas ações da empresa nas reviews de seus jogos o fato do Mirage, que eu ainda não joguei por ser muito caro simplesmente o jogo foi lançado e eu não ouvi falar dele, é sério eu não sei como é o Mirage. Não, eu não queria uma 2B de protagonista de um Assassin's Creed (eu amo AC mas comparar NieR:Automata com qualquer outro jogo é heresia, obra prima atemporal), e também não queria um Guts de protagonista (porque assim como a 2B eu jogo AC pra aprender história e curtir história, não pra chorar em posição fetal com total desesperança a realidade humana e o futuro sombrio deste mundo [quem sabe, sabe]) só queria que a Ubisoft respeitasse a etnia do povo que ela está contando a história, como ela sempre fez. Antes de um fã, eu sou um consumidor, eu não coloco minha mão no fogo por quase ninguém na indústria de jogos (Yoko Taro e Kazutoki Kono são as únicas excessões), mas se uma empresa multimilionária vai lá e cobra 600 pila no jogo, que é metade de um salário que eu ganho, e espera que eu aceite certas decisões equivocadas na parte deles, eu não vou ficar quieto, minha opinião importa para eles numa escala de consumidor, mas acredito que a Ubisoft arranjou um problema desnecessário, eu não via uma pressão tão grande desde o anúncio do Cod Infinity Warfare. Eu torço para o sucesso do jogo, estamos no mar porque recente em que teve tanta coisa boa de Japão feudal, Shōgun, a série da FX, é maravilhosa fazia tempo que uma série não me prendia tanto, Sekiro é perfeito e Ghosts of Tsushima é lindo, mas não vou negar que estou esperando o pior, até pro jogo me surpreender, caso eu consiga comprar. |
