2024.05.20 06:33 Ratzor24 Relationship Between Serotonin, 5-HT2A Receptors, and KCNQ2/3 Channels

  1. KCNQ2/3 Channels and Neuronal Stability:
    • KCNQ2/3 (KV7.2/7.3) channels contribute to the M-current, which stabilizes the resting membrane potential and controls neuronal excitability.
    • Proper function of these channels is crucial for preventing hyperexcitability.
  2. Impact of Serotonin on KCNQ2/3 Channels:
    • 5-HT2A Receptor Activation: Increased serotonin levels can activate 5-HT2A receptors, inhibiting KCNQ2/3 channels and reducing the M-current, leading to higher neuronal excitability.
    • Inherent Dysfunction: If KCNQ2/3 channels are not functioning properly, increasing serotonin can exacerbate hyperexcitability.
  3. Effects of SSRIs on Neuronal Excitability:
    • Initial Phase: SSRIs increase serotonin, initially activating 5-HT2A receptors and inhibiting KCNQ2/3 channels, which can cause side effects like anxiety and agitation.
    • Long-Term Adaptation: Chronic SSRI use leads to desensitization and downregulation of 5-HT2A receptors, eventually normalizing KCNQ2/3 channel function and reducing excitability.
  4. Consequences of Abrupt SSRI Discontinuation:
    • Withdrawal Symptoms: A sudden drop in serotonin can destabilize receptor activity and KCNQ2/3 channel function, causing withdrawal symptoms and increased excitability.
    • Gradual Tapering: Slowly reducing SSRI dosage can help mitigate these effects and provide a smoother transition.
  5. Therapeutic Implications of KCNQ2/3 Channel Activators:
    • Stabilizing Excitability: KCNQ2/3 channel activators (e.g., retigabine) enhance channel activity, maintaining neuronal stability despite serotonin level changes.
    • Combining Treatments: Using KCNQ2/3 activators alongside SSRIs can help manage side effects and improve treatment outcomes.
KCNQ2/3 channels play a vital role in stabilizing neuronal excitability. Serotonin increases can inhibit these channels via 5-HT2A receptor activation, leading to hyperexcitability. While SSRIs initially increase excitability, long-term use desensitizes 5-HT2A receptors, restoring balance. Abrupt SSRI discontinuation can disrupt this balance, causing withdrawal symptoms. KCNQ2/3 channel activators can counteract these effects, stabilizing excitability and improving patient outcomes when combined with SSRIs.
Long story short serotonin can Mess with KCQN2/3

2024.05.20 02:03 i_eat_ass_all_day I feel like nothing is changing and everything sucks :(

Hey all, sorry to be the next depressed poster on this sub.
I'm really sorry because I know you guys probably get annoyed by these posts.
TL:DR - I will probably never be able to live as a woman because nothing is happening and I don't have a way to fix it.
I'm a 23 y/o transfemme.
I've been on hrt for a year and three months now and I have had zero changes. I've talked with my friends and I have pictures from just before and throughout the last year and three months - nothing's is different as far as I can tell or as far as my friends can tell.
I guess my HRT levels aren't good? I got checked every month when I started until 4 months in and now it's every three months. My first doctor that I started with never said whether or not my levels were specifically too low or anything except for my first test with him, which made sense since I was on low dose mono therapy. My new doctor always say something is wrong with my levels, telling me that they were way too high and my dosage was way too high and now she keeps saying that they are too low after she reduced and modified my dosages. I want to trust my new doctor because she is widely regarded in my area as the best hrt doctor. As for now, I recently started injections and have been on it for 4 weeks.
I also worry that it may just be because my body is going to change with my genetics. Even as a man, my genetics have cursed me with awful traits like REALLY wide shoulders, big hands and big feet. And I know that cis women also can have large feet and broad shoulders and big hands but I can bet that 99% of cis women don't wear a size 14M/15.5W shoe. I also started having my hair thin in My freshman year of high school, so that's cool - a year and half of Finasteride hasn't done shit for that. There is also my facial hair, no matter what I do - I always cut myself when shaving, No matter what. I've been trying to get into a dermatologist for months now to see if I have some fucked up skin or something because it's getting to the point where I often find my self crying because I hate the feel of my stubble. Same with body hair, my skin always breaks out into hives and rashes when I shave my body, no matter what cream and razor I use.
My friends and (most of) my coworkers refer to me as a woman and use she/her pronouns but I can't help occasionally feel contempt as it feels like they are mocking me almost. I know they aren't but it definitely feels like they are. And for the coworkers that doesn't do that, they often look at my she/her pronoun pin and then proceed to use them/them for me, and according to my coworker friends - just use he/him when I'm not around.

2024.05.19 22:12 White_Ace_of_Spades The King Has No Clothes - Part 1 (Short Story)

Author's Note: Welp, I'm back. I'm going to be doing these super-short stories from now on instead of the multi-paged short stories that took several days to write. I tend to lose motivation in writing when I have long, drawn out stories. I just want to write these stories about my characters and focus on their character beats and emotions, rather than one long drawn out plot. I do intend on making an overarcing plot with these stories, but told over many, smaller instances.
This story introduces a new character that will not be recieving a character concept page. The reason why is because I simply am not interested in making game concepts and I'm not interested in Overwatch (The Game) anymore. I just want to focus on the stories of my characters.
Harvey King, the handsome, smooth-talking, knuckle-bruiser of Overwatch, now reduced to another ruffian locked behind steel bars. The once highly-esteemed son of New York Cities biggest tech conglomerate entrepenuer, Leonhard King, now another criminal locked away for life for the high crime of terrorism. Not only that, but he was thrown under the bus, used as a scapegoat to get pressure off of King Enterprises, the company that dad always loved more than his own son. His own son, dammit!
"I'm tellin you Harv," Sean muttered on from the top bunk. "you've gotta cool off or you'll get yourself killed!"
"Shut up!" Kingbursted out at his insolent cell mate. The New Worker was a fairly handsome man with a sharp jawline and beautiful nose, but his charcoal black hair had become ragged due to a profound lack of hair gel in prison. Harvey was looking into a glass shiv that he had stolen from the one-eyed punk down the isle, desperately trying to get the angle just right so that he could see his reflection in it. He ran is his fingers down his jaw line, feeling the creases and folds that had been left by Winston's Tesla Cannon. His perfect face, the face who's nose had never once been broken in all of Harvey's days of brawling, was now left with an electrical scar that he would have to live with for the foreseeable future, at least until he can break out of this hole and get it fixed with a healthy dosage of surgury. "That damn monkey, he ruined my face!" He turned to Sean. "Look at this, he ruined my face!"
"Yeah, I've been looking at it for the past month or so." Sean snickered.
"You realize I already have a life sentence?" King snarled, his tone had grew contempted.
"Yeah, so? Most people here d-"
"'That means it won't matter if I use your teeth for golf balls!"
"Oh please kid, you ain't the first young punk to threaten me li-"
"I'll be the first one to go through with it," King yelled at Sean. grabbing him by the collar and dragging him down from the top bunk. "If you don't shut your fu-"
"Shut up in there!" A guard banged his baton against the bars, interrupting King. "And get your hands off your cellmate!"
"Sorry officer," King put on a happy, personable tone of voice for the gaurd. He obediently dropped Sean. "Won't happen again, I promise."
"Yeah, yeah." The guard walked off, uninterested in King's crocodile smile.
The moment the guard turned his back, King's face switched like a lightbulb, going from bright and charismatic to dark and irritated in a mere moment. He turned back to his stupid cellmate and lashed out at him, grabbing his collar and pulling him in close. "I think we need to come to an understanding, you and I," Harvey spoke menacingly. "Look pal, I've already had to put up with a lotta bull lately. I do not need a dishevelled, ugly piece of crap, such as you, giving me more bull. So if you continue to irritate me, I'm gonna make you choke on your teeth. You get me?"
Sean, at a lost for words, simply nodded out of fear. Sean was ugly and dishevelled, Harvey was truthful when he said that, but he was also much smaller than Harvey and wouldn't be able to defend himself.
"Good." Harvey gave Sean a toothy grin. Even in the dimly lit, murkey prison cell, Harvey's teeth still glistened. He dropped Sean, who immediately crawled back onto the top bunk, hugging the wall this time as to avoid being pulled down again, and sat down on his cold, hard matress. He took a deep breath and closed his eyes, trying to think of how to get out of this situation. Only he couldn't think of an escape plan, he had too much on his mind. He was trying to make sense of everything that's happened, the mistakes he made that lead to this point. He had everyone at Overwatch wrapped around his fingers like marionettes, his facade was perfectly crafted. He couldn't help but reminisce about when he first started, the conversation he had with his father to get the whole thing set up, the developing of squeaky-clean persona that would be irresistable to Winston and Sojourn, the persona that landed him a spot on Overwatch's small roster of agents and made everyone fall in love with him. Well, not literally fall in love with him, but he was a natural smooth talker and very easy to like upon first meeting.
He remembered the conversation with his father that got him introduced to the likes of Winston in the first place, the conversation between his father and the leaders of Overwatch. King Enterprises had promised to provide Overwatch with funding, but only so Harvey could get a chance within their ranks. Leonhard King, father of Harvey King, was the CEO of King Enterprises. The old man was an idealist, and so was his only son. They wanted to leave a good impact on the world, with King Enterprises having been nicknamed "The Company of the People"; at least, that's what the public saw. No, the real Leonhard King was a tyranical capitalist who valued money over human rights, a greedy old man who was born into a mafia family, but being the the second-born meant he never got a chance to take over the family when his father died. Instead, it was his elder brother, Everett King II, named after his and Leonhard's father, who got to take over the family. Unsatisfied with being second fiddle, Leonhard left the family completely to become an entrepanuer, and now, 50 years later, he was on top of New York's social hierarchy with the biggest tech conglomerate in New England. King Enterprises was built into many facets of life in America, they built all sorts of cars, home appliances, home security systems, but that's only what the media saw.
Of course, Leonhard never shook off his knack for crime. He was a King, and criminality flowed through the King Bloodline; taking money out of decent folks pockets was in their blood. The horrible truth of the matter was that King Enterprises, "The Company of the People", got into all sorts of shady business, such as illegally shipping arms overseas, racketeering, maintaining ties in the black market, and lobbying shady politicians. On top of that, King Enterprises was a major player in the Military Industrial Complex, producing all sorts of weapons for the government, and shipping all the surplus over seas to other countries; a good chunk of all equipment used by the U.S. military was manufactored by King Enterprises. And it is because of this influence on the Military Industrial Complex that Leonhard King also provides funding to terrorist organizations such as Talon, organizations who are keen on creating conflict and pushing the world to the edge of war. Corporate corruption ran through every level of King Enterprises, and everyone outside of the corporation was none the wiser.
It was yard time, and King was hanging out alone. He hadn't made any attempts to socialize with any of the other ruffians the populate the prison, for he isn't planning on staying very long, but he did intend on working out as much as he could while he was there. A long time ago, when King was a kid hanging out in the slums of New York, he had a buddy called Twig. He was called Twig because he was very skinny, but mostly because King wasn't a very imaginitive nicknamer. He got sent to prison at the age of 16 for accidentally killing a guy while trying to jack his car, and came out a few years later completely jacked. King ended up shooting and killing him a while after he got outta prison, mostly because Twig blamed King for getting locked up and wanted revenge, but King still remembered in high regards.
King was readying himself for the bench press, stretching and giving himself a mental preptalk, thinking along the lines of "just cuz you're in prison doesn't mean ain't King no more," and the like. While he was getting ready, a loud, gruff voice cut off his train of thought. "Hey, punk!" King swung his head around to see who was yelling at him, and beheld a man who was around his same height, but very, very prison-jacked. Not as prison-jacked as Twig was, but still very prison-jacked. "Your daddy wanted to give you a message." This man didn't seem particularly friendly.
"My daddy?" King was not happy about this. A message from his father? Really? That old bastard wanted nothing to do with his son, and if he wanted to send him a message, he could visit him on every other thursday! "The hell's the meaning of this?"
"He said you couldn't be naming any name, kid. Said you'd understand." The big man pulled out a shiv, swinging his arm forward while rushing at King.
"The hell!?" King exclaimed, caught off guard by the sudden attack. He managed to catch the man's wrist, stopping the shiv from plunging right into his guts. He had to act quickly, he wouldn't be able to hold this man's arm for very long before he got overpowered, so he had to resort to some dirty tactics. Spitting in the man's eyes then kicking him in the shin did the trick, causing the jacked man to faulter and drop his shiv. Harvey had been in many a street fight during his life, and with the shiv out of the equation he would be also to out-box this man. King was an experienced brawler, he learned how to tussle on the streets of New York City, and was once mentored by a prize-fighter how to box. If not for his father's interference, King would've spent his entire life living in slums, hanging around his Uncle's turf, instead of in the King's Estate with his father.
With his attacker instinctually wiping spit out from his eyes, King had the perfect opportunity to land his signature KO'ing right hook right into the man's dome. King wasn't the biggest, most intimidating fella on the block, but he knew how to throw a punch with a surprising amount of power. King threw several more punches, beating the man senseless with merciless intensity, just to let off some steam that he's been holding in since his incarceration. He would've beaten that man to death if he hadn't been hit in the shoulder by a gaurd's heavy baton from behind. King fell down and got tackled by a gaurd, ending the fight.
Thursday rolled around, and it was the other thursday. King, of course, didn't have any visitors, but he did have a phone call. King impatiently tapped on the tabletop, eagerly waiting for someone to pick up the phone. A wide, sparkling smile stretched across his face as the wheezy voice of an elderly smoker come out the phone. "Who is this?" The voice calmly asked.
"Hey, Uncle."

2024.05.19 19:56 DryCheesecake1129 Rhinophyma How to diagnose early stage

Hi, So, now i can say for sure i got rhinophyma. I think i will say something about my nearly 2 years journey and how all started. It is very hard to find something on the internet, so i hope it can be useful for some people. Well, so first my problems with redness on face started 15 years ago. I was misdiagnosed with sebderm, later with allergies, but never with rosacea. After 2, 3 years i gave up on all creams and started enjoing my life with sun. And boom, all redness disaperad, at summer my face was looking normal, per winter little bit red but it was not so bad. And then after 10 years, so 2 years ago i wake up one day with a swelling on my left cheek, and pain in this area. I have extremely sensitive skin and can feel even smallest changes. I went to GP and because redness was very small they said it is not dermathological.I was sure from begining it is some kind of phyma, but....i went to whole procedures. First CT scan and ENT visit...all clear. Then MRI, also shows nothing. It was september 2023 when i developed big butterfly red rash on my cheeks. And finally was directed to Dermathology. First doctor said he doesn't see nothing and this type of redness is normal, i could not believe it. Luckly i made revisit and seen another doctor. He examined me and said it is rosacea or lupus, but more like rosacea, prescribed me with ivermectin, metro and doxycyline 40 mg, also has done bloodtest for ANA and ENA to eliminate lupus. Tests came back negative but medications was not helping. At february they done biopsy from my cheek. It was the time( january, february) when i started to notice shape change on my nose. It bacame more boulbus, larger and more bumpy with more thickened skin on right side.At january i mentioned it to my derm but he started laughing and said for sure it is not rhino. Well, again doctors who are not listening patients, they do not understand that patient can see the changes and feel them. But whatever, my biopsy results came back and three weeks ago i had a visit with new derm.Biopsy eliminated lupus but shows skin thickening, and i again mention about my nose, she examinated it( first doctor who touch it) and said, yes skin is more thicker at this places, there is also diffrent skin texture and colour. It is early rhinophyma, she sent me for bloodtest to start isotretinoine, and after 2 weeks i got visit with diffrent derm, if the bloodtest will be ok and he will diagnose rhino as a second opinion i am starting isotretinoine/accutane. I am devastated. My mental is so low that i am afraid of mirrors, i reduced social contacts to minimum. But....couple more sympthoms that i got: 2 years ago on my nose developed two small tiny red spots, it never dissapeared. In this 2 years more of them appeared, always 2 close together. They not disapearing. Too small to squize them, even if i tried there was only blood. Feeling pressure on my nose all the time, right side is bigger and i can feel it is heavier. Itchy mostly on the tip of the nose. Of course bulbous shape, tip is larger, left side at the bottom, right side from the middle of the nose to the bottom, whole nose looks bigger. Oily skin, large pores on the nose. Redness, or more pinkish, more like big pink spots, not whole nose is pink, where the skin is overgrowing it is more like blindspots and skin is tough, when i touch it i can feel like a lizard skin. And the best, sympthom that i never found on the internet but maybe someone got it as well: I can hear, feel some cracking, poping sounds from my nose. First i thought it is from inside, but now i am sure it is skin overgrowing making this sound, maybe because it is pressing airtubes inside the nose, i do not know, but it is horrible. I wake up at night and can't sleep because of that sound. Ok, long post, but i tried my best to mark all the sympthoms.Hope it will help someone else to diagnose it asap. Good luck.

2024.05.19 12:23 jaxonjason Journal on the Importance of Treating Drug-Induced Psychosis as a Short-Term Diagnosis

Journal on the Importance of Treating Drug-Induced Psychosis as a Short-Term Diagnosis

Introduction

Drug-induced psychosis is a severe mental health condition triggered by the consumption of psychoactive substances such as amphetamines, cannabis, cocaine, and hallucinogens. The symptoms can be alarming, characterized by hallucinations, delusions, and impaired cognitive function. Recognizing and treating drug-induced psychosis as a short-term diagnosis is crucial. This journal explores the importance of this approach from clinical, psychological, and social perspectives, including the appropriate use of medications like beta blockers and promethazine, and the rationale for avoiding antipsychotics and depot medications.

Clinical Perspective

1. Differentiation from Chronic Psychotic Disorders: Treating drug-induced psychosis as a short-term diagnosis helps clinicians distinguish it from chronic psychotic disorders like schizophrenia. Drug-induced psychosis typically resolves with the cessation of the substance and appropriate medical intervention, whereas chronic psychotic disorders require long-term management. Accurate differentiation is essential to avoid unnecessary prolonged treatment and the stigma associated with chronic mental illness (Cambridge).
2. Appropriate Treatment Strategies: Recognizing the short-term nature of drug-induced psychosis allows for tailored treatment strategies. Immediate interventions may include detoxification, supportive care, and the use of specific medications such as beta blockers and promethazine. Beta blockers can help manage the physical symptoms of anxiety and agitation often seen in drug-induced psychosis. Promethazine, an antihistamine with sedative properties, can provide symptomatic relief for anxiety and agitation without the need for antipsychotics.
Use of Beta Blockers:
Use of Promethazine:
Avoidance of Antipsychotics:
Dangers of Depot Medications:
3. Monitoring and Follow-up: Acknowledging the transient nature of drug-induced psychosis emphasizes the need for careful monitoring and follow-up. Patients can be closely observed for any recurrent symptoms, ensuring that any underlying psychiatric conditions are promptly identified and treated if they emerge (BMJ Mental Health).

Psychological Perspective

1. Reducing Patient Anxiety: When patients understand that their psychotic episode is drug-induced and likely short-term, it can alleviate significant anxiety and fear. Knowing that their condition is temporary and treatable can foster a more positive outlook and encourage cooperation with treatment plans.
2. Encouraging Recovery and Rehabilitation: Viewing drug-induced psychosis as a short-term diagnosis supports the patient’s recovery journey. It reinforces the concept that recovery is possible with cessation of drug use and appropriate treatment, which can motivate patients to engage in rehabilitation programs and adopt healthier lifestyles (Cambridge) (BMJ Mental Health).
3. Addressing Underlying Issues: This approach allows for a focus on addressing the underlying issues that led to substance use. Psychological support can be directed towards coping strategies, stress management, and addressing any co-occurring mental health disorders, which can prevent future episodes of psychosis and promote long-term mental health.

Social Perspective

1. Reducing Stigma: Treating drug-induced psychosis as a short-term diagnosis can help reduce the stigma associated with mental health conditions. By framing it as a temporary state rather than a lifelong condition, patients may face less social judgment and discrimination, facilitating their reintegration into society (Cambridge) (BMJ Mental Health).
2. Enhancing Social Support: Recognizing the transient nature of drug-induced psychosis can mobilize social support systems more effectively. Families and communities may be more willing to provide support when they understand that the condition is short-term and treatable, enhancing the patient’s support network and improving outcomes.
3. Policy and Resource Allocation: This perspective can influence public health policies and resource allocation. Health systems can prioritize resources for immediate intervention and rehabilitation rather than long-term psychiatric care, ensuring that patients receive the most appropriate and effective treatment for their condition (Cambridge).

Conclusion

Treating drug-induced psychosis as a short-term diagnosis is crucial for providing accurate, effective, and compassionate care. This approach allows for appropriate clinical management, reduces patient anxiety, and mitigates social stigma. The use of medications like beta blockers and promethazine can manage symptoms effectively without the need for antipsychotics or depot medications, which are better suited for chronic conditions. Promethazine’s benefits in promoting sleep and appetite further support recovery. Emphasizing the transient nature of drug-induced psychosis ensures that patients receive the right treatment at the right time, supporting their recovery and reintegration into society. Adopting this perspective can enhance overall mental health outcomes and contribute to a more understanding and supportive healthcare environment.

2024.05.19 10:32 Ok-Mountain-7176 Spiro stopped working it seems

So I must fist say that I am in my early thirties and ave gone through accutane when I was eighteen and that god rid of a lot of my acne. And that I have been using tret since December 2022 . And my skin is mostly clear except around my period I would get like three pimples or the odd one . But all in all I don’t have j’huge issues. I started tret 0,05 too in December 2022 to mprove texture and pores. I started spiro April 2023 because I was having unusual breakouts that I think were linked with a contraceptive pill that I stopped anyways. And I loved it so much because I had no side effects and no breakouts at all. And even if they were not a lot I would get very upset when I would get one or two like social anxiety. It’s been a few months like two months it seems to not be working as well as it used to. I started having my period again which I didn’t in the beginning for four months but the breakouts came back . Not bad but I have a little around my period. I must also mention that I started at 150 because my dermatologist told me to. So I can’t really up my dosage. Should I stop ? Like maybe my body got used to it . And there is no point now. Or is it still working but I am getting greedy ? I don’t know now because the tret also improves my skin. But during the summer I had nothing no acne at all ever and no period which was annoying..

2024.05.19 08:47 _MFB_ RADS - Reactive Airway Disorder Syndrome, A type of asthma

I've been dealing with what was initially diagnosed as asthma after inhaling toxic fumes from heated Motul 300V POWER 5W40 motor oil from the exhaust system of a Subaru we were working on back on March 19, 2019. This caused severe symptoms like excess mucus and a drowning sensation. Recently, my doctor has diagnosed it as Reactive Airways Dysfunction Syndrome (RADS). I also have asthma and allergic rhinitis. Motul 300V POWER 5W40 contains several components that can be hazardous when inhaled: lubricating oils (petroleum, C20-50, hydrotreaded neutral oil-based) 10-25%, mineral oil 1-2.5%, distillates (petroleum, solvent-dewaxed heavy paraffinic) 1-2.5%, and C14-16-18 alkyl phenol <1%. These components can release harmful fumes when heated, leading to respiratory irritation and other health issues.
After the exposure, I underwent several tests in April 2019 to assess the impact on my respiratory health. The arterial blood gas (ABG) test on April 4, 2019, showed normal acid-base status with a pH of 7.415, pCO2 of 40.2 mmHg, and pO2 of 89.8 mmHg, with oxygen saturation at 97.1%, indicating no immediate respiratory failure but highlighting the need for further assessment. The pulmonary function test (PFT) with post-bronchodilator study on April 4, 2019, showed normal spirometry results with no obstructive ventilatory defect noted. The FEV1/FVC ratio was within normal limits, and there was no significant response to bronchodilators, suggesting that the primary issue was not related to airflow obstruction. The chest X-ray on April 2, 2019, showed some degree of haziness in the right lower lung, which could be due to vascular shadows or beginning/resolving infiltrates, with no consolidation observed. My recent chest X-ray shows all clear, indicating that any previous abnormalities have resolved, suggesting no current radiographic signs of lung disease or persistent inflammation.
On May 10, 2024, I cut my Symbicort dosage from 2 puffs AM and PM to 1 puff each. I felt good after missing my dose on May 16, 2024, so I reduced it to 1 puff a day. As of May 17, 2024, I'm off Symbicort completely. During a Stryd run test meant to last 10 minutes, I ended at 4:40. No wheezing, headache, or chest tightness—just felt out of breath and had some phlegm, but didn’t need to use Symbicort. Regular use of POWERbreathe has really strengthened my respiratory muscles. I'm successfully managing asthma without daily Symbicort, keeping symptoms in check. Overall, I'm feeling better with improved breathing and less reliance on meds. Using the POWERbreathe IMT device has been a game-changer for me. It’s allowed me to reduce and eventually stop using Symbicort. My respiratory muscles are stronger, and my asthma is under control.

RADS - Reactive Airways Dysfunction Syndrome
• Definition: RADS is a condition that resembles asthma but is distinct from it. It occurs after a single, high-level exposure to an irritating substance, such as fumes, chemicals, or smoke. This exposure leads to acute and chronic respiratory symptoms. • Relation to Asthma: While RADS presents with symptoms similar to asthma (such as coughing, wheezing, and shortness of breath), it is not classified as a type of asthma. Instead, it is a specific syndrome triggered by an acute exposure event, whereas asthma is typically a chronic condition with multiple potential triggers.
Correct Description
RADS - Reactive Airways Dysfunction Syndrome
• Definition: A condition that occurs after a single, high-level exposure to an irritating substance, leading to asthma-like symptoms such as coughing, wheezing, and shortness of breath. • Relation to Asthma: While it has symptoms similar to asthma, RADS is distinct and is specifically triggered by a one-time exposure to an irritant, unlike chronic asthma which can have multiple and ongoing triggers.
This distinction is important for understanding the nature of RADS and how it differs from chronic asthma.

2024.05.19 05:08 Significant_King1494 Stretching Vials with Orderly

If I order larger doses with Orderly than I administer to make it last longer, will they keep me at the same dosage when I refill? Just wondering if they would consider this a pause necessitating a reduced dosage or starting over. Stretching 15 mg doses to last 3 months at 5 mg is appealing, but will I still get 15 mg prescribed after a two month delay in renewing? TIA!

To date, what has been the most intense government strategy to contain the drug problem in the U.S.? Have we ever done anything similar to Switzerland?
Recently stumbled into this post and it sounded intriguing.
I apologize if I’m asking something that everyone here already knows, so please feel free to recommend books and other materials on the history of fighting the drug problem in the U.S.
Here, aside from offering methadone, suboxone and rehab, and aside from legalizing marijuana in certain areas, has the government ever attempted to be the main “supplier” of clean drugs as a strategy to combat illicit drug trading/dealing?

2024.05.19 00:44 As_A_Feather 1.5 months progress (40mg per week microdose)

So I really had no intentions of taking progress pics because I'd gotten so used to avoiding makeup-free selfies at any cost (even if they were just for me to see). But I just couldn't believe this was actually my skin when I looked in the mirror today and felt like I just had to document it. I'm nearly 37 and I can't believe I let myself suffer for 24 years before embracing accutane.
Apologies for my goofy looking mouth in the first set (I was biting my tongue in concentration, haha).
The second set of photos were taken about six weeks ago, right before taking my first dose (at my dermatologist's urging). This was after most of my foundation had melted/flaked off my skin midway through the day (as it did most days), but overall, I had actually considered that a great skin day as I had no active cysts. Still, my skin was so congested, inflamed, and angry all the time, even when it was in "healing" mode. Basically, as long as I had no active, painful explosions, I considered myself blessed. I would say my acne was mild-moderate but maddeningly persistent, and it just always looked...unhealthy? There was nothing I'd tried up to this point that managed to yield anything but the most temporary of results (and trust, I have tried EVERYTHING under the sun).
I was determined to microdose from the beginning, and am planning on staying on it for as long as I can—ideally lowering to a maintenance dosage of 10-20mg a week. I've only taken a total of 40mg a week for the last six weeks (10mg on Tuesdays, Wednesdays, Fridays, and Saturdays). I take it with a big spoonful of peanut butter and a claritin. I also take NAC and Milk Thistle for extra liver support. Plenty of water, obviously. I've honestly had ZERO side effects that I'm aware of—not even bothersome dryness, really. Though I do keep myself properly slugged pretty much whenever I'm home. My topical routine is exceedingly simple and gentle.
Topical Routine:
PM—double cleanse with baby shampoo, apply about a quarter-sized pour of pure vegetable glycerine to very damp skin and let it air-dry, then slug with vaseline
AM—splash with cold water, mix a nickel-sized pour of vegetable glycerine in my wet hand with a small shake of l-ascorbic acid powder from The Ordinary, mix it well, apply it all over my still damp face, and let air dry before GENEROUS globs of SPF 50 everywhere that isn't covered with clothing.
No makeup but lip gloss and a bit of mascara occasionally (makes SPF re-application easier).

One Year In And I Could Not Be More Happy!
I started Spironolactone about a year ago! When I started, I had painful hormonal acne around my chin and on my cheeks that had come back with vengeance after a year on Accutane. For the first three months, I started with a smaller dose of 50mg. After not seeing significant improvement, I upped my dosage to 100mg. After a month on 100mg, my skin began to clear up! Now, a year later my skin is very clear besides the occasional pimple. I was so nervous to start Spironolactone, but now I am so grateful that I did. I am now very confident and happy with my skin!

2024.05.18 23:20 Ambitious-cow-2971 How does your medication help you?

I’ve tried all brands and they kind of affect me the same but not in a good way.
I take 30mg , I always feel nauseous, stomach runs, and body doesn’t really feel normal, it feels more jittery, and what confuses me the most is I don’t feel any more focused while on the medicine? I just feel super awake, like no need for sleep and I guess that’s the amphetamine.
I told my doctor about this, that it doesn’t make me feel focused, just super awake, and that’s when I was on 40mg so he reduced the mg to 30mg and gave me a brand that doesn’t have the stimulants.
Then he also gave me prescription for anxiety medicine loll, never told him I had anxiety but he said they’ll help me as well , I haven’t tried them yet cause i was really confused why he’d prescribe me that when I don’t have anxiety and I really hate drugs.
I’m not consistent with my medicine, but mostly cause it doesn’t do its job… or maybe I’m misinterpreting what I’m supposed to feel so that’s why I’m asking
Do you feel more focused when you take the medicine? Right now I’m having trouble even concentrating on this tv show I’m watching and I really like it… I feel super jumpy, can’t focus on one tasks at all, mind just jumping frm one thing to another like I just feel more amped up to jump from one thing to another than stay focused on one thing …
And I can’t ask for a higher dosage because this sht really be making me feel naseaus asf and like not normal lol, even my mom says my eyes look like I’m on drugs when I was on the 40mg loll which amphetamine is a drug so I really am on drugs lol
Would love all input

2024.05.18 20:29 undercovermeteor Head Canon: Dimitri is medicated by Rhea

I've seen discussions on why Dimitri is/isn't so plagued by hallucinations that he loses all reason in different routes but I've always been surprised that no one offers what seems to me to be the most obvious answer - Rhea is medicating him to keep him docile.
Why do I think Rhea is medicating Dimitri? Well it wouldn't be the first time she used medicine as a means to temper emotions. In WC on all routes iirc, she offers a quest in which she asks you to distribute special herbs to several students she deems as "troubled". I believe Ashe (who just lost his adoptive father) is among them - and given Lord Lonato's ties to revolutionaries it would be detrimental to her if Ashe became emotional enough to lash out against the Church.
So if Ashe, and I believe Marianne and Annette (?) are big enough security risks for Rhea if they are ill at ease, it's almost certain that Dimitri is too. If she is as observant as Jeralt says she is then she could easily have recognised Dimitri's darker side just as Felix did and began medicating him early on. Since it's revealed in his support line with Ingrid that he has little to no sense of taste, it wouldn't even be hard to do so without his knowledge.
When the war starts in AM, VW and SS, Rhea and Dimitri are separated. His rapid decline in all three routes would align with his removal of access from emotion-stabilising herbs without an appropriate reduction period after prolonged use. Before long follow auditory and visual hallucinations and violent impulses, the likes of which had never appeared so intensely during his time at the Academy but had appeared to some extent in his childhood, if Felix is to be believed.
In a CF route, however, he stays with Rhea during the war. If he is not still being medicated five years later, she would have at least been able to properly manage reducing dosages until he had basically been staved off of it since she never disappeared. It would explain why CF is the sole route in which he hasn't fallen to a level of mental instability in which he can no longer be reasoned away from his constant bloodlust.
So yeah. That's my head canon

2024.05.18 20:20 Ok-Mountain-7176 Spiro stopped working it seems

2024.05.18 19:50 Ok_Dimension4846 Ipamorelin- Great add to your GLP1 journey!!

Ipamorelin- Great add to your GLP1 journey!!
How does Ipamorelin work? Source link
Ipamorelin is a synthetic variant of the hormone ghrelin naturally occurring in the body. Growth hormones and other hormones, such as insulin-like growth factor 1, are produced in response to ghrelin.
Growth hormone levels naturally decrease as we age. Ipamorelin stimulates the production of growth hormones by targeting specific receptors. This increase in growth hormones results in the following:
  • Increased muscle mass
  • Improved bone density
  • Decreased fat mass
  • Improved skin tone
Ipamorelin is also helpful in boosting metabolism, regulating appetite and energy levels, enhancing cell regeneration, and improving overall well-being.

Possible benefits of Ipamorelin

As a synthetic growth hormone-releasing peptide (GHRP), Ipamorelin is believed to have numerous health benefits. It has potential benefits in general health, anti-aging, and weight loss.
Here are some of the most notable ones:
  • Bone anti-aging. Researchers have found that Ipamorelin promotes bone cell formation in rats, thereby reducing bone deterioration due to aging.
  • Reduced aging impact on muscles. Animal studies have shown that Ipamorelin increases muscle mass, strength, and endurance. Physical performance could be improved by reducing the impact of aging.
  • Increased weight loss. In addition to reducing fat mass, Ipamorelin preserves muscle mass, resulting in a healthier approach to weight loss.
  • Better cognitive function. Since Ipamorelin stimulates the release of growth hormones, known to improve memory and learning in animal models, cognitive benefits may also be associated with Ipamorelin. Ipamorelin may enhance its immune system functions overall.
  • Boosted immune system. Ipamorelin may enhance its immune system functions overall by improving the body's ability to respond to infection and inflammation.
Despite Ipamorelin's potential benefits, it should be noted that the evidence is from animal studies, not solid medical evidence. Understanding how Ipamorelin peptides work and how they can help you before beginning treatment is essential.
Dosage
Ipamorelin is typically administered from 3 mcg to 10 mcg a day. Any treatment regimen should be discussed with your doctor before beginning since individual factors can affect the dosage.
When administered properly and under doctor supervision, Ipamorelin may provide significant results and improve quality of life.

2024.05.18 18:46 CBBlyes Medication Working Too Well. Advice?

Hello,
I could really use some advice, because I’m not too sure what to do at this point in treatment and feeling a bit hopeless.
Quick backstory, I’m a 23 M about 175 pounds. My acne was never bad, but I suffered from constant whiteheads, inflammed bumps, and the occasional cyst since 15. I truly fit the “recurrent and unresponsive to other treatments” reason for prescribing Accutane. I started in December 2023 and dosage (mg) to date has been 40:60:80:80:80:60. The drop from 80 to 60 is why I’m posting.
The Accutane worked instantly at 40; within a week all my acne disappeared and I didn’t get another pimple until halfway through 60. It was magical. Of course I had dry skin, lips, eyes, but nothing even close to outweighing the benefits or what I had heard from others’ experiences. First month on 80 was when everything changed. I got 3 cysts in a matter of a month, facial skin got extremely dry, rashes on hands and arms, skin started peeling off toes and bleeding (especially when working out), back pain, nose bleeds, beard hairs/follicles bleeding etc — the works. I kept pushing but eventually enough was enough and I reached out to my dermatologist who prescribed me a month of Prednisone. I had never taken a steroid in my life up to that point, but instantly 95% of the side effects subsided and all inflammation from the cysts disappeared.
Fast forward to last month, I ran out of my Prednisone and most side effects came back, and even more intense. But to my surprise, after my first time shaving my beard (I use straight razor, 2 blade, shaving cream, with grain, once weekly) I got the worst razor burn and bumps. I tried OTC Hydrocortisone and Aloe, but it didn’t work. I gave it a week, but no relief. I reached out to my dermatologist who prescribed Triamcinolone Acetate (topical steroid cream). Again, weeks of application and some relief, but still noticeable irritation.
What’s bothering me now, is my beard is so painful. I try to let it grow and not touch it, and it’s almost as if Eczema is occurring underneath and the beard hairs feel like needles; but if I do shave, the “Eczema” subsides but the bumps become more obvious (pictures 24 hours ago).
I feel like a shell of myself. My acne is flawless. I haven’t had a pimple in months and it’s obvious the medicine has done a fantastic job.. but possibly too well.
Any advice? I plan to reach out to my dermatologist tomorrow. I want to quit the Accutane early, but I’m scared of relapsing. I’m tracked for 9 months and have almost completed 6. The decrease to 60 was my request, and I don’t know when/if I’ll see a difference in the side effects. I can handle almost all the side effects throw my way so far, but the beard crossed the line.

2024.05.18 17:12 7e7en87 Anybody taking OptimALA version of NA-RALA(most bioavailable R-lipoic acid)

My sixth bottle came yesterday from ND. Was taking 3 capsules daily for first 2months, now 2 capsules(2caps would be reccomended dosage and one month stash as it has 60 capsules).
This supplement did what lyrica()pregabalin never come close to do stopping nerve pain(pins and needles, burning sensations, cold sensations and like ants crawling through my whole body). I realize also my tinnitus dissapered completely and eye floaters especially on clear sky and white backgrounds is gradually depleted.
OptimALA is mix of NA-RALA which is most bioavailable natural R-isomer of lipoic acid with a highly specialized starch called Cyclodextrin. By loading R-ALA into these cyclic starches, R-ALA stability is drastically improved, as is the bioavailability. Also third component is sesame seed extract that is very high in two sesame lignans called sesamin and sesamolin. The combination of alpha lipoic acid and sesamin has been shown to enhance each other effects, especially when it comes to fat loss and the optimization of our metabolic process.
Taking NA-RALA deplete thiamine so taking also fat soluble B1 called Benfotiamine is smart move and B1 is most important B vitamin for nerve health and normal glucose levels.
As I realize and read from studies R-lipoic acid is only antioxidant soluble in fat and water that passes blood brain barrier and chelate mercury, iron etc.other heavy metals, toxins., slow brain atrophy in MS by regenerating glutathione, vit.C, vit.E and Coq10, also new studies confirm that R-lipoic acid increase NAD levels.
Suplements that also can boost glutathione like NAC and sulforaphane I never felt anything, maybe something from Curcumin bcm-95 version but nothing significant,
It is being tested for its ability to boost myelin repair in MS as it lowers several markers of inflammation and reduces the number of immune cells that migrate into the brain and spinal cord and damage the myelin coating of nerves.
Main problem is with short 3hour half life of lipoic acid and R-S plain ALA is highly unstable, that is improved by NA-RALA, but OptimALA improved that even further with other two compounds that blend perfectly with NA-RALA.
I was taking before ALA 600mg, ALA sustained 600mg, NA-RALA and only with NA-RALA like 500mg seeing some progress. OptimALA is on whole another level as it has best bioavailability by far and clearly it is doing something.
Best way to take it is 2hours before or after food.
I truly believe this supplement is a wonder for brain and nerves. Our body produce tiny amounts of lipoic acid and when we age(also like CoQ10) levels are dropper from around 35 years old age and toxins and heavy metals are gradually contributing to this even before.
Red meats, particularly organ meats are the highest food source of Alpha Lipoic Acid. With that, broccoli, spinach, beets are other great sources too.
When I buy supplements I want best quality(3party lab tested) and what is in the vein with research.(bioactive compounds and to get highest bioavailability).
This is only supplement that I would highly reccomend even in front of mushroom extracts and some peptides.

2024.05.18 15:05 karmarevange CPA or Bicalutamid? Let's debunk myths on breast development stall and lack of muscle atrophy linked with the former treatments respectively

TLDR:
I started taking CPA one month ago and got scared about the rumors of CPA stalling breast development. Therefore, I started considering to switch to Bicalutamid, but then I also found rumors of muscle development not being reduced or even being increased under bica regymes. I want to take an informed decision of what is best for me and documenting this information for people with the same doubts in the future.
Q1. Do you have anecdotal experience about these two Myths? Q2. Do you know about more formal evidence, like studies on this regard?
++++++++++++++++++++++++++
Here's a summary of what I've read so far in many subreddits and studies.
Muscle atrophy under HRT (and Bicalutamid):
Breast development stall under CPA: - Some studies show that progestins at an early development stage, do objectively stall breast growth, as the breast proceed to create alveoli which can jeopardize the development of milk ducts by turning them into a terminal stage too early, however those studies are based only on rats and the dosages were very high. - CPA is a progestinr, however transfem patients consume it in rather low dosages, and are usually not rats. -There is variety of examples and counter examples in the subreddits for breast timelines. -There are success stories with CPA, but it's hard know if those individuals were genitcal outliers, and we cannot tell if results could have been even better without introducing progestins so early - Q4: Do you have statistics or further evidence of CPA stalling breast growth?
+++++++++++++++++++++++++++
Sources:
(1)... (I'll add links to the sources when I am at home with my laptop)
(2)....
(3)...
(4)... ....
+++++++++++++++++++++++++++++
My personal story:
I was on low dose E monotherapy for the first months which was not very effective, and started recently including CPA.
My body is mesomorphic and my genes give me muscle mass very easily. I've been tryng ineffectively to loose muscle mass for almost two years by consuming low protein and reducing exercise.
In the last weeks with CPA and a very agressive calorie defficit I have seen finally some progress but I'm very afraid to loose it when switching too Bicalutamid, however I do not want to jeopardize my breast growth on the long run.
Q5. Would you swiitch to Bicalutamid in my position, considering that reducing upper body muscle mass is very important for me, considering that breast development is of course a huge source of euphoria for me too.
I would be grateful to read a response to any of my five questions 😊
P.S.: As you might have noticed I have no clue of human biology, medicine, farmacology, biochemestry or microbiology, and of course no formal background on those topics, and any feedback about misconceptions I have made, false impressions and corrections are very welcomed and I'll edit it for future readers.
